Footprints is a registered provider of services under the NDIS. We offer services tailored to meet your needs and the supports funded in your NDIS Plan. If you have an NDIS plan, you can contact us about putting your plan into action and about the services, you may need. We offer you the options, the choice is yours.

Before you meet with your NDIA planner, it is a good idea to start thinking about your needs, goals and aspirations. The planning process covers all areas of your life, so it is important to give yourself plenty of time to think this through.
The NDIS is a federally funded scheme that puts you at the centre of the decision making process about how you want to live your life. The NDIS will provide all Australians under the age of 65 who have a permanent and significant disability, including psychosocial disability (term used to describe disabilities that may arise from mental health issues) with the reasonable and necessary supports they need to enjoy an ordinary life.
The NDIS will put more control and decision-making in your hands, and Footprints can work alongside you to support you to meet your needs and goals. NDIS will open up more opportunities and as a registered provider, we can assist you to prepare for and understand the NDIS.
Age
You must be under 65 years of age when you apply to join the Scheme.
Residency
• be an Australian citizen OR
• hold a Permanent Visa OR
• hold a Protected Special Category Visa, that is you were in Australia on a Special Category Visa on 26 February 2001 or had been in Australia for at least 12 months in the 2 years immediately before 26 February 2001 and you returned to Australia after that day.
Disability
You may meet the disability requirements if:
• you have an impairment or condition that is likely to be permanent (i.e. it is likely to be life long) and
• your impairment substantially reduces your ability to participate effectively in activities, or perform tasks or actions unless you have:
• assistance from other people or
• you have assistive technology or equipment (other than common items such as glasses) or
• you can’t participate effectively even with assistance or aides and equipment and
• your impairment affects your capacity for social and economic participation and
• you are likely to require support under the NDIS for your lifetime.
• An impairment that varies in intensity e.g. because the impairment is of a chronic episodic nature may still be permanent, and you may require support under the NDIS for your lifetime, despite the variation.
Support Coordination
We provide ongoing coordination and capacity building support as well as complex and specialist coordination support. We will draw on our extensive experience of local knowledge and coordination to assist you to interpret your NDIS plan in a way that meets your needs and ensures you get the best value for money.
Therapy Services
Footprints provides assessment, support and training to assist you to improve independence, such as Occupational Therapy, Social Work, and Community Nursing Services. Footprints also provides individual and group counselling to facilitate emotional acceptance and growth, and to assist you to gain personal goals and greater insight into your life. Social support groups are also available.
Light Street Centre – Centre Based Activities
Art in the Park, Music Group, Internet Café, Around the Ridges (social outings in the community), Fantastic Fridays (for people looking for friendship and peer support)
Daily Life
Assistance with self-care activities, house cleaning and other household activities tailored to improve daily living and enhance independence of daily tasks. This may also involve completing essential household tasks if you are unable to do this yourself. Footprints can also provide preparation and delivery of meals if you need support to do this.
Social & Community Participation
We can support you to participate in recreational or social activities. This support includes activities in Footprints Light Street Centre, and/or in the community, such as individual and group social outings and activities. Individual and group support is available to assist you to participate in activities of your choice, such as support to attend music concerts, the Theatre, sporting events, theme parks and other recreational activities.
Accommodation & Housing Support
Assistance in obtaining and maintaining housing, support to connect you with key housing providers, decluttering services (including direct support in your home, and individual and group psychological therapies), and support to help you meet your tenancy obligations, as per your tenancy agreement.
Learning & Skill Development
Individual and skills development groups are available to enhance your independence: Cooking Skills Group, Man-made Collective, Community Pocket Garden, Individual Community Participation activities are available including mentoring, peer support and individual skill development for a range of identified goals.
Footprints Social Support Groups have a range of activities for people residing in both South and North Brisbane.
Our programs are designed to develop, maintain and support social interaction, independent living, health and well-being. Providing people with the opportunity to develop friendships, make social connections and community participation. Some examples of activities at our centre include games, quizzes, exercise programs,themed events, Ekka, Melbourne Cup, talks and visits from health professionals and community organisations.
These groups can be funded through the National Disability Insurance Scheme (NDIS) or Queensland Community Support Services (QCSS).
Allied Health
Footprints offers a range of allied health services including a monthly podiatry service (appointments required) at the Light Street Centre. We also offer in home and centre based diversional therapy.

Providers of attendant care and community participation supports who meet the eligibility criteria have access to a higher price limit through a Temporary Transformation Payment (TTP). This will assist providers to continue transforming their businesses in the move towards a more competitive marketplace.
Footprints charges at the price limit in the NDIS Support Catalogue for all supports.
Underpinned by person-centred values, the FDS team creates a personalised action plan with the individual through a holistic approach, addressing needs and concerns relating to hoarding, as well as other psycho-social issues people may be facing. FDS is fee-for-service, and also available through Home Care Packages/NDIS Packages.
FDS provides a psycho-social response to support the client’s needs and goals, aiming to improve their overall quality of life. FDS aims to increase individual skills and knowledge to improve independence and capacity to address both the causes and symptoms of hoarding behaviours.
FDS can also arrange for In-Home Support Workers to work one-on-one with individuals to directly assist with decluttering and organising.
