Care Finder Program
A service to assist older adults connect to aged care services and supports, care finders support older people who experience significant barriers to accessing services, and who could otherwise fall through the cracks.
Care Finder is a free service, fully funded through Primary Health Networks.
How Care Finders help?
Care finders work at a person’s own pace to understand their situation and support them through the necessary steps. They:
- Provide information about local aged care services
- Facilitate My Aged Care assessments to access support
- Identify targeted, available services to meet needs
- Support both initial service access and transitions when needs change
Who is this program for?
This program is for older Australians who need extra intensive support to navigate the aged care system. Care finders support people who:
- May have difficulty communicating because of language or literacy challenges
- Find it difficult to understand information and make decisions
- May be reluctant to engage with aged care or government services, or
- May be in an unsafe situation if they do not receive services.
To receive care finder supports, a person must be:
- Isolated and does not have a carer or support person they feel comfortable to act on their behalf, and
- Eligible for government-funded aged care.
Care finder is a free service, fully funded through the Primary Health Networks.
Service regions
The program operates from North Coast NSW through to North Queensland, as part of a national initiative.

- Northern NSW for First Nations First nations people,
- Country to Coast (Central Queensland, Wide Bay, Sunshine Coast),
