Commonwealth Home Support Program

Do you need assistance with tasks at home, or perhaps with travelling to appointments? The Commonwealth Home Support Program (CHSP) is funded by the Australian Government Department of Health and Aged Care and provides flexible and timely home care support.

CHSP offers a range of services, based on what you need to live at home for longer.

Based on your personal requirements, the CHSP team can support you with:

  • Transport to appointments
  • Personal care support
  • Exercise Physiology
  • Meal preparation at home
  • Social activities and connection with community
  • Assistance with Care and Housing (ACH) — hoarding and squalor support

And as your needs change, our services are also able to adjust to suit you.

Service regions

CHSP is available in the Brisbane North, Caboolture and Brisbane South Aged Care Planning regions.

Assistance with Care and Housing (ACH) supports are available across South East Queensland.

Who is the CHSP service for?

The Commonwealth Home Support Program is for people who are:

  • still living in your home,
  • 65 years and older, or 50 years and older for Aboriginal and Torres Strait Islander people,
  • AND in need of help to continue to live independently.

You can be referred to CHSP by contacting My Aged Care on 1800 200 422 if you are:

  • or 50 years or older for Aboriginal or Torres Strait Islander people.
  • 65 years of age or older
