Services we provide

Under the Support at Home program, we will provide a comprehensive range of tailored and discreet services delivered by a highly trained and dedicated team. Please remember, your services are paid for from the funding you are allocated and, therefore, are subsidised by the Government. What you contribute will depend on factors such as the type of service you are accessing, your chosen provider and your contribution rate as determined by Services Australia, based on your financial situation. If uncertain about what financial co-contribution you may need to pay towards your services, please talk to your Footprints Care Partner who can answer any questions you may have.

As a not-for-profit organisation, we aim to provide quality support and optimal value from your Support at Home budget.

Service locations

We service the Brisbane North, Brisbane South, Caboolture, and Logan regions.

We can provide you with personal care support, including assistance with:

  • showering
  • personal hygiene
  • grooming
  • dressing and undressing
  • getting in and out of bed
  • mobility within your home
  • medication taking
  • meal preparation and support with eating.

Remaining connected to your community is vital for health and wellbeing and preventing isolation and loneliness. We can support you to help you maintain your friendships and hobbies.

This can include support with:

  • receiving social visits in your home
  • attending recreational activities and groups (sporting, shopping and church)
  • visiting friends and family.

We provide housekeeping and cleaning support as well as gardening and yard maintenance.

This may include:

  • dusting
  • vacuuming
  • mopping
  • dishwashing
  • kitchen and bathroom cleaning
  • bed making and changing
  • washing and ironing
  • lawn mowing
  • weeding
  • basic garden maintenance

Under Support at Home, the new term for “care coordinator” or “case manager” is Care Partner.

As your care partner, we will assist you in coordinating your care/support plan and ensure you get the best possible outcomes and value from your support at home package.

We can provide nursing services to manage your health at home. Our nursing staff are registered nurses and can assist you with:

  • managing your medications
  • wound care and dressing changes
  • monitoring blood pressure
  • catheter care
  • oxygen care
  • health monitoring.

We can support you to stay physically and mentally fit and feel confident in the community and living independently at home. Supports we can provide include:

  • occupational therapy
  • exercise physiology
  • restorative care and support
  • home environment evaluations
  • recommendations for assistive equipment
  • training in activities of daily living

We provide in-home respite care for you so your carer can take a break or run errands and attend appointments.

Respite care can be arranged regularly, occasionally or for one-off support.

We provide transport to help you attend appointments and other commitments.

Your care partner can accompany you, if required, or simply provide reliable transport to drop you off and pick you up.

Support at Home provides older people with Federal Government funding towards accessing care and support to continue living at home independently. Once you have been assessed and assigned funding, you can choose an approved provider, such as Footprints Community, to arrange the services you need.

From November 2025, Support at Home replaced the previous Home Care Packages program.

Support at Home offers eight levels of support, meaning there is more funding available to those needing higher care. The highest level of support is up to $78,709 per person.

Additionally, the program offers upfront, short-term funding for:

  • home modifications and assistive technology
  • restorative care
  • end-of-life care.

Not sure what services can be accessed? The Australian Government website displays a Support at Home defined services list, offering greater clarity around what is covered by your support level.

The Support at Home program provides a quarterly budget to access services. If you don’t spend your quarterly budget, you can carry over a portion (maximum of $1000 or 10% – whichever is higher) of the unspent funds to the next quarter.

If you are a current Home Care Package client, your unspent funds will transfer with you to Support at Home in full.

How much you contribute depends on:

  • the type of services you receive
  • your income and assets
  • your pension status.

Clinical services: no contributions are made for clinical supports, such as nursing and physiotherapy. They are fully funded by the government.

Independence support: moderate financial contributions are required for services like personal care, and assistive equipment and products.

Everyday living support: a higher level of financial contribution is required for services like domestic assistance and gardening.

You will only pay contributions for services that you have received.

Your contribution rate (which will be a set percentage for each service type) is determined by Services Australia through an assessment of your income and assets and your support needs.

  • lowest contributions for full pensioners
  • moderate contributions for part pensioners
  • highest contributions for self-funded retirees.

If you are an existing Home Care Package client, you will keep your current funding level and will not have to be reassessed (unless your needs have changed). Your unspent funds will transfer with you to Support at Home in full. Your contributions will follow a “no worse-off” principle under grandfathering arrangements.

When you come to us, you will be assigned a Support at Home care partner.

Your care partner will take time to discuss your support plan with you to ensure you are matched with your preferred services. Your care partner will work with you and for you, answering any questions you may have.

The program is for people who:

  • are aged 65 or older (50 or older for Aboriginal or Torres Strait Islander peoples, or people who are homeless or at risk of homelessness).
  • need help with daily tasks such as personal care, household tasks, or getting to health services.

The level of support you can access will depend on your needs, which will be determined through a My Aged Care assessment.

  • Go online: Use the online application form on the My Aged Care website.
  • Phone: Call My Aged Care on 1800 200 422.
  • In-Person: Make an appointment to see an Aged Care Specialist Officer (ACSO) at a Services Australia location.
  • A trained assessor will contact you to arrange a time for the assessment, which is usually carried out at your home.
  • If eligible for Support at Home, you will receive a notice of decision and an individual support plan to share with your provider. At this stage, we welcome you to contact Footprints to get started in accessing services.

  • regularly review your services to ensure your needs continue to be met

Each Footprints carer undergoes National Police Check screening and regular training. Footprints has been providing support in community for over 35 years and is a well-known and trusted service provider. We currently deliver Home Care Packages and will assist our clients to transition to Support at Home from November.

The new Aged Care Act includes a Statement of Rights, outlining the rights that older people will have when accessing aged care services.

The Statement of Rights include the right for every individual to have:

  • independence, autonomy, empowerment and freedom of choice
  • equitable access
  • quality and safe funded aged care services
  • respect for privacy and information
  • person-centred communication and the ability to raise issues without reprisal
  • advocates, significant persons and social connections.

Footprints is committed to ensuring our actions are consistent with the Statement of Rights. This will include ensuring that their aged care services are delivered by aged care workers who have appropriate qualifications, skills and experience.
