Do you know an older person in the South Burnett, Somerset (West Moreton) or Lockyer Valley region who needs support with managing their health?
Healthy Ageing Connections is committed to supporting older people to improve their health outcomes at home.
This program is available to older people aged 65 years or older, or over 50 years if an Aboriginal and Torres Strait Islander person.

- Connect older people to available services
- Encourage independence and healthy living
- Strengthen relationships with family, friends and community
- Connect people with supportive local communities
- Improve quality of life and wellbeing

Somerset (West Moreton), Lockyer Valley, and South Burnett and Cherbourg.
Who is this program for?
This program provides support, services and information to people who are:
- Aged 65 years or older, or Aboriginal and Torres Strait Islander peoples over 50 years of age
- Experiencing a chronic health condition or one or more risk factors
- Frequently hospitalised or at risk of hospitalisation
- At risk of premature admission to residential care
- Requiring support to manage health in the community.
