Social Health Connect
Our team is here to help you feel more connected, supported, and well.
Are you feeling isolated or unsure about how to get involved in your community? Social Health Connect is a free service for adults aged 18+ who live in the Redcliffe, Caboolture or Kilcoy hospital catchment areas. The service is for people who have unmet social needs, impacting their health and wellbeing.
Program participants are paired with a dedicated Link Worker who listens, helps you set goals, and connects you with local social opportunities, community groups, or services that match your interests and needs.
Who is this program for?
Social Health Connect is for adults (aged 18 and over) who have unmet social needs which are negatively impacting on their health and wellbeing.

Social Health Connect is for people living in the Redcliffe, Caboolture or Kilcoy hospital catchment areas.
This program can help you:
- Overcome loneliness and social isolation
- Join local groups or activities
- Build confidence and reconnect with others
- Navigate barriers like transport, confidence or language
Our team members are called Link Workers, and they are skilled at building relationships. They are well connected to a range of groups and community services in Redcliffe, Caboolture and Kilcoy and have the knowledge to navigate government systems, if needed.
Social Health Connect takes a person-centred, strengths-based approach using a Social Prescribing model. The program offers an open referral pathway. We also strongly encourage referrals from GPs and health professionals.
Our team works with clients to link them into groups and social opportunities that are meaningful for them. We also work with clients around capacity building to address barriers to participation.
