Social Worker in GP Service
If you are living with an ongoing health condition, and are aged 18 or older, you can ask to speak to our social worker who is based at participating medical centres at Woodridge, in the Brisbane South region.
Our qualified social worker can help you manage your condition by offering practical supports, such as connecting you with community resources and services, coordinating your care services, and providing person-centred therapies.

Who is this program for?
This service is for people who are:
- 18 years of age or older
- Living with a chronic health condition
- Currently under the care of a GP or Nurse
- A resident of the Brisbane South region
Translation services are available.
Location
Brisbane South. Our social worker is based in the Smith Brothers Medical Clinic in Woodridge.
Our social worker is here to help you manage your health condition and to help improve your quality of life and wellbeing. It may be that you need practical assistance, more information or just someone to talk to for emotional support.
Our social worker provides:
- Initial assessment and intake
- Counselling and support
- Case management and advocacy
- Crisis intervention
- Care coordination
- Connection with community resources
- Person-centred therapies
