Footprints Community is one of a group of organisations selected and funded by the Australian Government to deliver aged care needs assessments under the newly established Single Assessment System.
Footprints provides this service across several regions in Queensland and Norfolk Island.
These include the Wide Bay, Sunshine Coast, Caboolture, Brisbane, Gold Coast, Logan River Valley, Darling Downs and West Moreton regions.
Since early 2025, Footprints’ Aged Care Assessment Service (ACAS) has been conducting:
- aged care needs assessments for in-home aged care, flexible aged care programs, residential respite and entry into residential aged care.
The Single Assessment System provides a single assessment pathway for older people to enter aged care and access different services as their needs change.
To access an assessment, you must first register with the Australian Government’s My Aged Care. You can do this online or by calling My Aged Care on 1800 200 422.
