Carmel’s Story | Enhancing independence and wellbeing

Carmel, 79 years old, lives independently in her home with her dog and cat. She has been managing her care needs but faces increasing challenges to her wellbeing as she ages.

Before connecting with Footprints, Carmel was on a Level 2 Home Care Package (HCP) but needed to move up to a Level 3 HCP due to her declining health and the onset of dementia. Difficulties with her previous service provider left her feeling socially isolated and without the assistance she needed. She also faced challenges navigating the complexities of the My Aged Care system and had concerns about her personal safety.

Carmel was referred to Footprints by her Mental Health Case Manager from Queensland Health. During 2023–24, Footprints provided Carmel with personalised and practical support. Courtney, from our Seniors Vitality and Health Connect (SVHC) program, played a key role in helping Carmel be reassessed for a Level 3 HCP. Courtney also organised a falls alarm to address Carmel’s personal safety concerns, arranged a dementia clock to help her manage her daily routines, and linked Carmel with a local dementia support group to provide social and emotional support.

When SVHC was discontinued, Carmel seamlessly transitioned to the Care Coordination Service and came under the care of our Senior Link Worker, Lloyd.

As a result of Footprints support, Carmel has experienced significant improvements in her wellbeing — her personal safety has been enhanced, her social connections have improved, and she has reduced feelings of isolation. The support she received has given her peace of mind.
