Who is this program for?

This program provides support, services and information to people who are:

  • Aged 65 years or older, or Aboriginal and Torres Strait Islander peoples who are over 50 years of age
  • Experiencing a chronic health condition or one or more risk factors
  • Frequently hospitalised or at risk of hospitalisation
  • At risk of premature admission to residential care
  • Requiring support to manage health in the community.

  • Somerset (West Moreton)
  • Ipswich
  • Lockyer Valley
  • Scenic Rim (West Moreton – includes Boonah)
  • South Burnett and Cherbourg.
